Reports To: Manager of Community Programs and Services
Classification: Full-Time, Non-Exempt
Location: Worth, Illinois (On-Site)
Schedule: 37.5 hours per week (9am
-5pm); occasional evenings and weekends as needed
Compensation: $21.54 - $25.64 per hour, commensurate with experience and qualifications

Position Summary

The Family Advocate provides case management, advocacy, resource coordination, and supportive services to individuals and families experiencing social, economic, behavioral health, housing, educational, immigration, or family-related challenges.

This position works closely with clients, community partners, and internal programs to promote stability, safety, self-sufficiency, and overall well-being through culturally responsive, trauma-informed, and client-centered service delivery.

Key Responsibilities
  • Conduct client intake, assessments, screenings, and service planning.
  • Provide ongoing case management, advocacy, and supportive services.
  • Connect clients with community resources, public benefits, healthcare, housing, employment, educational opportunities, and supportive programs.
  • Develop and monitor individualized service plans.
  • Provide crisis intervention, emotional support, safety planning, and referrals when appropriate.
  • Assist clients with applications, documentation, and follow-up activities.
  • Maintain accurate and timely client records and case documentation.
  • Enter and maintain information in agency databases and case management systems.
  • Track client outcomes and assist with program reporting requirements.
  • Conduct outreach activities and represent AAFS at community events, schools, health fairs, workshops, and partner meetings.
  • Build collaborative relationships with community organizations and service providers.
  • Participate in team meetings, training, and professional development opportunities.
  • Ensure compliance with HIPAA, agency policies, grant requirements, and professional ethical standards.
  • Perform other duties as assigned.
Minimum Qualifications
  • High School Diploma, GED, or equivalent.
  • One (1) to three (3) years of experience in human services, social services, community outreach, family support, case management, caregiving, or a related field.
  • Strong interpersonal, communication, and organizational skills.
  • Ability to work effectively with individuals from diverse cultural, linguistic, and socioeconomic backgrounds.
  • Basic computer proficiency, including Microsoft Office applications.
  • Ability to maintain confidentiality and professional boundaries.
Preferred Qualifications
  • Associate or bachelor’s degree in social work, Human Services, Psychology, Sociology, Public Health, Family Services, or a related field.
  • Experience working with immigrant, refugee, or underserved populations.
